What insurances do you take?
We accept most major insurances including Medicare, Blue Cross Blue Shield, MVP, Cigna, United Healthcare and Vermont Medicaid
We accept many other insurances as well. We encourage you to check the website of your insurance company to verify that our providers are in your network. If you have questions about whether or not we accept your insurance, please call us at 878-1008.
What types of services do you provide?
We are a comprehensive family medical practice. In addition to the preventive and chronic care services you would expect from a primary care office, we offer a wide range of other services including lab testing, minor procedures (skin tag and mole removal), laceration repair, casting and vasectomies. How do I obtain my medical records?
Much of the information contained in your medical record is available on your Patient Portal. If you are transferring care to another medical practice, you can fill out a MEDICAL RECORDS REQUEST FORM and we will send your record to your new practice free of charge. If you would like a copy of your medical record for yourself, you can fill out a Medical Records Request form and our medical records staff will provide you with a copy for a fee of $0.50 per page.
What if I’m sick over the weekend or after hours?
Evergreen Family Health is open on Saturday from 9 AM to 1 PM. We are open on Saturday specifically so that you don’t have to go to the Emergency Room or elsewhere for care. If you are sick after hours, we always have a physician on call who can be reached by dialing 878-1008 and choosing option 6.
If you are having a medical emergency, you should always call 911.
Do you have same day appointments?
If you are sick any day Monday through Saturday, we will see you that same day. We designate at least one practitioner to stay after hours and see all patients who are sick and need to be seen that day. You may not get an appointment with your regular primary care practitioner, but you will be seen on the day that you are sick.
Who do I call with billing questions?
For questions about your bill, please call our office at 878-1008 and choose Option 5 to be connected with a Billing Specialist who will be able to assist you.
What is your cancellation policy?
If you need to cancel or reschedule your appointment, please call at least 24 hours in advance so that we can use your appointment for another patient. Please be aware that we charge a no-show fee for missed appointments.
How do I get medicine refills?
Please plan ahead and request needed refills in person at any scheduled office visit. If you must request a medicine refill over the phone, please allow up to 48 hours for your request to be completed.
Do I need to fast before my appointment?
Unless your practitioner has specifically told you to fast for a visit, you don’t need to fast. That being said, if you anticipate needing cholesterol or blood sugar tested, please refrain from eating for at least 2 hours before your appointment.
Can I pay my bill online?
Yes, you can pay your bill securely online by logging into your patient portal account. Your last statement will show on the portal and at the bottom of the screen is an option to pay the outstanding balance online. Please note that any payments you’ve made since your last statement will not be reflected in the balance due on your patient portal.
What do I do if I have a form or letter that I need completed?
Please call the office if you have a form or letter that you need to be completed. We ask that you fill out the form as completely as you can before sending it to us. If the form requires an office visit we will let you know. If you need a letter, please provide us with a draft of what needs to be written in the letter. We will do our best to respond to your request in a timely manner.
Who takes care of me if I need to be hospitalized?
If you are admitted to the hospital, your medical care will be provided by the hospital employed physicians, otherwise known as “Hospitalists”. Please let them know you are a patient of Evergreen and they will notify us so that we can help coordinate your care. Although we will not be visiting you in the hospital, our nurses and your practitioner are regularly updated about your hospital care so we can take over once you are home.
