A PCP is necessary for every patient so that patients have a clinician to coordinate all aspects of care including providing health maintenance and acute visit care; and managing all incoming records, test results, consultant notes, and other documents. You may view information about OUR PRACTITIONERS and then call one of our offices at: Evergreen Family Health (Williston) 802-878-1008 or Alder Brook Family Health (Essex) 802-872-7100 to register and schedule your first appointment.
