Evergreen Primary Care at 28 Park Avenue HAS MOVED to 426 Industrial Avenue, Suite 130, Williston, VT 05495.

Evergreen Sports Medicine HAS MOVED to 426 Industrial Avenue, Suite 130, Williston, VT 05495.

The former Charlotte office (located in Shelburne), the Essex Junction office (Main Street in the City of Essex Jct) and the 28 Park Avenue Williston office are all permanently closed.
