Evergreen Sports Medicine continues to be open to new patient consultations, regardless of who you see for primary care.

Our Evergreen Sports Medicine team provides comprehensive services, including on-site x-ray, for efficient and effective care of musculoskeletal injury and exercise related concerns. 

To become a new patient with Evergreen Sports Medicine, please call the clinic at 802-878-1175. 

Please have insurance information and primary care physician information available at the time of your call.

Previous imaging on your injury (x-ray, MRI, etc.)? Please let us know ahead of time so that we can provide the most comprehensive care.

A PCP is necessary for every patient so that patients have a clinician to coordinate all aspects of care including providing health maintenance and acute visit care; and managing all incoming records, test results, consultant notes, and other documents. Familiarize yourself with our practice:

Please read the new patient WELCOME LETTER for an informative description of the services, policies and procedures of our office. The welcome letter covers topics such as after-hours care, what to expect if you are admitted to the hospital and how we handle refilling your medications. You may also want to read through the Patient Rights & Responsibilities as well as the Practitioner Responsibilities.

Please review our FINANCIAL POLICY for important information regarding payments, and canceling of appointments.

Transfer your medical records:

Once you have registered with us and scheduled your first appointment, please fill out the MEDICAL RECORDS RELEASE FORM and return it to our office. We make every effort to obtain your medical records from your previous health care provider prior to your first visit.

Your first visit:

Before your first appointment, please fill out the ADULT HEALTH DATABASE form completely and bring it with you to your appointment. Plan on arriving 10 to 15 minutes early to fill out any additional paperwork and complete the registration process.
