The Flu is here!

It is that time of year to think about stopping the spread of the Flu.

The best way to prevent the flu is to get a flu shot.  Our office still has some available so please call if you would like to schedule yours.

To help prevent the spread of flu:

  • Cover you cough.
  • Wash your hands often and well.
  • Keep yourself healthy with rest, exercise, and eathing healthy foods.
  • Drink plenty of fluids.
  • Stay home if you get sick.
  • Avoid close contact with sick people.

  • Exciting News Regarding Your Health Management!

 We are proud to inform you that Evergreen Family Health’s Patient Portal now offers the power of internet access to manage the most important aspects of your healthcare.  The Patient Portal enables our patients to communicate with their healthcare team easily, safely, and securely via the Internet, and to take a more active role in their health care.

Participating patients are given secure User IDs and passwords, enabling them to access the Patient Portal to:

  •  view lab and diagnostic test results
  • receive email reminders for upcoming appointments
  • request and cancel appointments
  • ask questions on the Patient Portal instead of calling the office
  • view and print your personal health record, including immunization records and medication lists

Lyme Disease

It’s Lyme Disease prevention time again.

Lyme disease is caused by infection with Borrelia burgdorferi, a type of bacteria called a spirochete. The infection is transmitted by the bite of a black-legged tick, known as the deer tick.  In most cases, an infected tick must be attached for at least 36 hours in order to transmit Lyme disease.

Do I need to be seen?

  • In most cases, if the whole tick has been removed and was on for less than 36 hours, the answer is no.
  • If  you were not able to remove the whole tick or if it was on 36 hours or longer, then, yes you should call for an appointment.

New Medication and Pharmacy Procedures

We have launched a new initiative for the purpose of safety, efficiency and quality of care in managing your long term medications with you. We realized it would be nearly impossible to safely and efficiently handle the high demand we saw for telephone renewals.  Each request requires a review of medical records, with assessment of accurancy and current clinical status. Our renewal of medications often did not correspond with office visits, and medications ran out at differing times.

Evergreen Family Health prefers to NOT manage medications over the phone, as it is potentially unsafe.

Hospital Care Changes

With the advances in office based care, most hospitalized patients today are very ill. In the unfortunate situation that you require admission to a hospital, you likely will require several specialist consultations and ancillary services such as physical or respiratory therapy, or will have a condition that may require attention every few hours. Many hospitals including Fletcher Allen now have “Hospitalists” who are dedicated to coordinating the specialized care necessary in a hospital setting.

Because we want you to have the best care possible during your hospital inpatient stay, we will no longer be providing that care ourselves after September 6, 2010. We certainly will help coordinate your admission when necessary, and assist in transitioning your care back to the outpatient setting. Electronic health records allow us to stay in touch and remain up to date on your condition while you are hospitalized.
We look forward to focusing on what we do best through expanding Evergreen office hours, increasing staff, and bringing you additional outpatient family medicine services. Please note also that we will continue to provide newborn care in the hospital and Dr Reiss will continue to deliver babies at Fletcher Allen!

Renovations

If you have been at Evergreen Family Health lately, you may have noticed the work we have been doing. This is partly long overdue repainting and refurbishing, but we are also renovating to make our space more efficient. We are creating team rooms for our practitioners and support staff to work together. Through this design, your physician will have a dedicated nurse and coverage practitioners who can get to know you better.

We also look forward to creating a computer station in the reception area for your reference and convenience during your time here. In addition, we will be adding some exam rooms, and a new nursing room. Enjoy our new color scheme!

Barb Walker – Retired after 20 Years!

Barb Walker – Retired after 20 Years!

In 1990, Barb Walker LPN and Dr. Paul Reiss began work at the current Evergreen site from the ground up while working with CHP Health Plan. Many changes throughout the years have brought us to a private independent practice of 4 Doctors, 2 Nurse Practitioners and a Physician Assistant. We have cared for thousands of patients and experienced some very difficult transitions in the past twenty years. Through it all, Barb Walker kept track of all of you and all of us. She retired in June. She looks forward to moving to Montgomery, VT to be closer to her grandchildren and enjoy a well deserved time of leisure!

We all miss her positive, up-beat attitude, her amazing willingness to embrace change – including and especially the Electronic Record – and her quirky and (sometimes) off-beat humor. Barb often has been the “glue” of the practice, and has witnessed the arrival of many newborns, as well as the passing of many of our dearest long term patients.
